The Municipality of Cuyapo in coordination with the Department of Foreign Affairs (DFA) Regional Consular Office will hold a mobile passport service on December 18, 2010. Application forms are available at the office of the Municipal Mayor.
Requirements (photocopy only):
- Authenticated Birth/Marriage Certificate
- 2 valid IDs
- Old Passport for RENEWAL
- P1,200.00 Processing Fee
Important Dates:
Nov. 15, 2010 - Last day for submission of application & requirements
Dec. 18, 2010 - Processing Day (7AM - 3PM)
Jan. 08, 2011 - Releasing Day
